The word microneedling gets used to describe a surprisingly wide range of treatments and devices. Professional clinic machines. Handheld motorised pens. At-home rollers. Micro infusion stamps. They are all grouped under the same general umbrella, but the mechanisms, the depths, the outcomes, and the appropriate settings for each are genuinely different.

 

This post breaks down each category properly. What each one actually does at a tissue level, who it is designed for, what it costs, and where it fits in a realistic skincare routine. Not to make the case for one over another, but to give you enough information to make an informed decision about what your skin actually needs.

 

I have personal experience with most of these. Some of it was excellent. Some of it was expensive. Some of it taught me exactly what I did not want to replicate. All of it eventually led to building Erra Skin.

 

Professional Radiofrequency Microneedling

This is the most advanced category and the one furthest from anything you can replicate at home. Radiofrequency microneedling devices, the most widely known being Morpheus8, combine two distinct mechanisms in a single treatment: microneedling and radiofrequency energy delivery.

 

Here is how it works. Insulated needle arrays penetrate the skin to a programmed depth, typically between 1mm and 4mm depending on the concern being treated. At the point of penetration, the device delivers radiofrequency energy directly into the dermis and, at deeper settings, into the subdermis. The combination of mechanical puncture and thermal energy creates a wound healing response that targets not just the surface of the skin but the deeper structural layers responsible for firmness, elasticity, and scar tissue remodelling.

 

 

Because the needles are insulated, the radiofrequency energy is delivered at the tip rather than along the shaft. This means the thermal effect is concentrated at depth rather than burning the surface. The result is a treatment that can address concerns that surface-level treatments simply cannot reach: deep acne scarring, icepick scars, significant loss of firmness, and skin laxity.

 

WHY RF MICRONEEDLING IS DIFFERENT:  Standard microneedling creates a mechanical wound healing response. Radiofrequency microneedling creates both a mechanical and a thermal response simultaneously. The thermal component at depth triggers a remodelling process in the structural layers of the skin that mechanical needling alone does not achieve. This is why it produces results for scarring and laxity that other treatments cannot match.

 

This treatment requires topical anaesthetic because the depth and the thermal energy make it genuinely uncomfortable without numbing. Recovery varies but typically involves redness, swelling, and sensitivity for several days. It is a medical-grade treatment performed by trained clinicians. It is not something you do at home and it is not something you do frequently.

 

I had radiofrequency microneedling twice in my early thirties, after my acne extractions, specifically targeting the scarring that had been left behind. My skin had to be numbed before each session. The device went deep. It was not a comfortable experience. But the results were real. My skin looked genuinely different afterwards. Smoother, more even, the kind of skin I had been trying to get back to ever since. That was back in 2018 and I was paying $680 per treatment. And even at that price I could only justify it twice. That is the reality of clinic treatments for most of us.

 

Radiofrequency microneedling is a genuinely effective treatment for the right concern. If you have significant acne scarring, icepick scars, or meaningful skin laxity, it is worth having a consultation with a clinician about whether it is appropriate for you. But it is a once or twice a year treatment at best for most people. And consistency, not intensity, is what produces lasting skin health over time.

 

Professional Microneedling Pens

Below radiofrequency microneedling in the clinical hierarchy sits the microneedling pen. These are motorised handheld devices used in clinic settings, as well as in cheaper versions sold for home use. The needle cartridge sits at the tip of the device and the motor oscillates the needles in and out at speed, typically between 6,000 and 18,000 cycles per minute, while the practitioner glides the device across the skin.

 

 

Clinic-grade pen treatments operate at depths of 0.5mm to 2.5mm depending on the concern. At these depths, particularly above 1mm, the treatment is working in the dermis and producing a meaningful wound healing response. Results for surface concerns like fine lines, uneven texture, and mild pigmentation are well documented.

 

The mechanical issue I have with pens, even in a clinic setting, comes down to the gliding motion. Because the device is moving laterally across the skin while the needles are oscillating, the needles are not entering and exiting at a truly perpendicular angle. The faster the gliding speed, the more pronounced the lateral component. In skilled hands this is managed. In less skilled hands, or at home, it becomes a scratching mechanism rather than a controlled puncture.

 

I have had a microneedling pen used on my face in a professional clinic setting. I paid $350 for that treatment. And I remember clearly feeling the drag of the device across my skin. It was not painful, but it was not the clean stamping sensation I expected from a needling treatment. If I had known then what I know now about the difference between a pen mechanism and a stamping mechanism, I would have asked different questions before booking. I did not have active acne scarring at that point. The pen was treating general skin quality concerns. And honestly, for that, I am not sure the gliding mechanism was the right tool.

 

Microneedling pen treatments in clinic typically cost between $200 and $500 per session depending on the provider and location. They are more accessible than radiofrequency microneedling but still not something most people can maintain as a consistent monthly routine. And consistency, as I will come back to, is the variable that determines long-term results more than any individual treatment.

 

THE PEN MECHANISM:  Motorised oscillation at speed creates hundreds of punctures per second while the device moves across the skin. Each individual needle entry has a lateral component because the device is in motion. In trained hands this is controlled. The concern is not with professional pen use in skilled clinical settings but with the translation of this mechanism to home use, where technique, pressure, and speed are uncontrolled variables.

 

At-Home Derma Rollers

The derma roller is the oldest and most widely available at-home needling device. A cylinder of needles rolled across the skin creates punctures through a rolling motion. No motor. No oscillation. Just a physical rolling action.

 

 

The fundamental mechanical problem with a roller is that needles cannot enter and exit perpendicularly when the delivery mechanism is a rolling cylinder. The needle enters at an angle, drags through the tissue as the roller moves forward, and exits at an angle on the other side. This is an ablative injury, meaning it involves the destruction and tearing of tissue rather than controlled displacement. The skin heals from this, and some renewal happens as a result, but the process is uncontrolled and the recovery is less predictable than a stamping mechanism.

 

The serum absorption issue compounds this. Because the wounds created by rolling are angled tears rather than clean vertical channels, they close unevenly. Most of the serum applied after rolling sits on the surface rather than being delivered into the skin at any meaningful depth.

 

Derma rollers are widely available from a few dollars to a few hundred. The lower price point drives trial but also drives disappointment, because the mechanism is not well suited to producing the consistent results it is marketed to deliver.

 

I tried a derma roller myself. I could not bring myself to press it hard enough to actually get to the 0.5mm depth it was designed for. It just felt too strange, rolling it across my skin like that. So I was doing it lightly, which meant I was not getting the depth, which meant I was not getting the result. And I do not think I was alone in that. The sensation of the rolling motion instinctively makes you pull back.

 

That instinct makes sense. The rolling mechanism creates a drag sensation that your skin correctly reads as something to resist. A stamp does not produce that response because the motion is straight down and straight back out. There is nothing to pull back from.

At-Home Micro Infusion

Micro infusion is inspired by professional microneedling. The mechanism that distinguishes it from both pens and rollers is the stamping action. The device is pressed straight down onto the skin, the needles enter perpendicularly, and the device is lifted straight back off. No gliding. No rolling. No lateral component. The same stamping motion of the radio frequency or Morpheus8 machines.

 

 

What this creates is an atraumatic micro-channel. The tissue is gently displaced rather than torn. The channel walls remain intact. And because the serum is delivered simultaneously through the needle mechanism as the channels are created, the active ingredients travel directly into the skin at the moment it is most receptive to absorption.

 

At 0.5mm depth, the Erra Skin device is working in the epidermis and the very upper dermis. This is a cosmetic depth, not a clinical one. It is not going to address deep icepick scars or significant skin laxity. What it is designed to do is support the visible appearance of healthier-looking skin through consistent, repeated sessions that compound over time.

 

The distinction between cosmetic and clinical depth matters and I want to be honest about it. Micro infusion is not a replacement for radiofrequency microneedling if you have significant scarring or structural skin concerns. Those require clinical depth and clinical expertise. What micro infusion offers is something different: a consistent, accessible, at-home practice that keeps the skin in genuinely good condition between clinic visits, or instead of them, for people whose primary concern is skin quality rather than structural repair.

 

I was sick of waiting to be rich to start looking after my skin better. That is the honest version of why I built Erra Skin. I was 37, I could see my skin changing, and the time was now. For skin quality concerns, the compounding effect of consistent repeated treatment outperforms the intensity of infrequent clinical sessions. For scarring depth and thermal energy is the correct choice. This is not a comparison of which treatment is better. It is a recognition that different concerns require different approaches, and that accessibility determines consistency.

 

Which One Is Right for You

The honest answer is that it depends on what you are treating.

 

If you have significant acne scarring, icepick scars, or meaningful skin laxity, radiofrequency microneedling is worth a consultation. It is expensive and it requires professional delivery, but for those specific concerns it produces results that nothing else matches. Do it when you can. Do not wait until you can do it consistently, because for most people that day does not come.

 

If you want general skin quality improvement, reduction in the visible appearance of fine lines and texture, better hydration, and skin that looks consistently well, micro infusion at home done fortnightly is the more sustainable answer. Not because it is better than clinical treatment in absolute terms, but because you will actually do it. And showing up consistently is worth more than the occasional perfect session.

 

If you are considering a microneedling pen at home, understand the mechanism before you start. The gliding motion requires technique that takes practice to get right. Used incorrectly it creates lateral tearing rather than controlled puncture. If you want to needle at home, a stamping mechanism is safer and more forgiving in an untrained hand.

 

The best skincare treatment is the one you can actually maintain. Intensity without consistency produces results that fade. Consistency without intensity produces results that compound.

A Final Honest Note
